Chronic Care Management (CCM) is a service intended to help patients with chronic diseases and health conditions learn to understand their condition and live successfully with it. RN Staffing Solutions is proud to offer CCM alongside our various services with uncompromising quality, effectiveness, and satisfaction.
CCM service is extensive, including structured recording of patient health information, maintaining a comprehensive electronic care plan, managing transitions of care and other care management services, and coordinating and sharing patient health information timely within and outside the practice.
